Annual Health Checks:
To ensure that learners are invited for their annual health check, they need to be part of the Learning Disability Register. The is a record of people aged 14 and over with a learning disability who are registered with a GP practice and gives them access to a free annual health check, which includes producing a health action plan.
A learners’ GP practice should contact the learner or their parent/carer either by letter or phone to make an appointment for their health check but parents/carers or learners can also contact their GP to request one if necessary. If a parent is not sure whether their child is on the register, they can ask the receptionist at the GP practice.
The College asked that parents/carers share the summary report or the health check or the health action plan with us so that we can include this as part of their next Education Health and Care (EHC) plan annual review. This information is vital to ensure that we are continuing to meet their health needs.
